Paulden Community School

every child - every chance - every day.

School Lunch Program Description

Paulden Community School believes that all students learn better when they have eaten throughout the day. Offering healthy choices is also an important reason why our school participates in the National School Lunch Program (NSLP). This federal program assists schools to provide healthy foods and make it available to all students. Because Paulden Community School takes part in this program, there are guidelines that must be followed, and we ask that our families cooperate with the process. Following these guidelines allows the school to ensure that all students are offered a healthy meal for breakfast and lunch. Free language assistance, auxiliary aids, and/or accommodations are available upon request.

Medical Statement for Students with Special Dietary Accommodations
Paulden Community School participates in the School Nutrition Program. Therefore, we are required to serve breakfasts, lunches, snacks, and milk that meet meal pattern requirements.

Paulden Community School has the option to make meal substitutions for those who make a request for reasons such as religious or moral convictions, or personal preference as long as the meal pattern requirements can still be met.

Paulden Community School is required to make meal substitutions for children with a disability that restricts their diet if a written medical statement from a State licensed healthcare provider is received.  If these meal substitutions cannot be accomplished within the meal pattern, meal substitutions can only be made by Paulden Community School once they have received a written statement from a State licensed healthcare professional who is authorized to write medical prescriptions.

The medical form contains two parts:
Part 1 is to be completed by the parent/guardian of the student needing the special dietary accommodation; and
Part 2 is to be completed by the state licensed healthcare professional.
